Class 1 – Who we are and what we do

Practice details, information about our Practice team and our opening times are all published on our website. This information can be accessed via the About Us page. 

Class 2 – What we spend and how we spend it

The Practice receives money from NHS Wales according to its contract for national General Medical Services in exchange for services provided for patients.

This information can be obtained on request in writing to the Practice Manager. The Practice Manager can be contacted at enquiries.fishguard@wales.nhs.uk

There may be circumstances where material cannot be released because it is confidential or commercial information or the appropriate officer designated for these purposes, under the Act, has taken the view that it may be prejudicial to the conduct of the Practice’s affairs.

If this is the case, we will respond to your FOI request with a formal letter acknowledging the reasons why we are unable to give you this information.

Class 3 – What our priorities are and how we are doing

  • Working closely with our North Pembrokeshire Cluster colleagues to develop services for patients across North Pembrokeshire.
  • Working towards achieving required standards within the Welsh Government QAIF (Quality and Information Framework), Access Standards, Medicines Management, Patient Safety, CGPSAT, IG toolkit and Healthier Wales.
  • The practice has not had an inspection for a while but is probably due to be inspected by HIW (Health Inspectorate Wales ) in the near future. This report will then be published here.

Class 4 – How we make decisions

The Practice has a committee that is made up of all the partners, salaried GP's, practice manager and nursing team.

The Committee meets every four weeks. 

Management policies are formulated at regular business meetings. Decisions affecting the provision of NHS services are recorded. These are available on written request (any information which is commercially sensitive or falls under the Data Protection Act is excluded). 

Written requests should be forwarded to Fishguard Health Centre, Ropewalk, Fishguard, SA65 9BT or by hand delivering a written request to our Reception Team for the attention of the Practice Manager. 

Class 5 – Our policies and procedures
